'Marine Envenoming and Poisoning Management' and 'Snake Bite Envenoming Management'

Remote Envenomation Consultancy Services (RECS) is 'on-call' consultation services and training provider for healthcare providers in management of bites, stings and envenoming from venomous animals or poisoning from toxin. The objective of the service is to optimizing and advocating appropriate treatment at every level of clinical management to enhance favorable outcome of patients. RECS Consultants consist of Emergency Physicians who are members of Malaysian Society of Toxinology (MST).

I was given the opportunity to attend their popular seminar which are 'Marine Envenoming and Poisoning (MEP)' and 'Snake Bite Envenoming Management (SEM)'. The seminar objective is to increase awareness and knowledge on the optimal and appropriate management of snake bite and marine envenoming and poisoning in the pre-hospital and hospital settings. The 2 days seminar was held at Dewan Selingan Institut Latihan Kementerian Kesihatan Malaysia (ILKKM) Sandakan which located near Hospital Duchess of Kent, Sandakan.

On the first day of the seminar, we cover MEP. SEM was cover on the second day of seminar. The lecture include differentiation of the term 'venomous' and poisonous' in which many commonly use inappropriately. 

Venom can be interpret as complex substance (can be enzyme and protein) produced by specialized gland (venom sack) and be deliver to target via specialized apparatus (fangs or stings). It commonly use for digestion or defense mechanism of animal. Venomous animal can be eaten. Example would be snake, stingray, spine fish, stone fish, jellyfish, anemones, cone snail, octopus, sea urchin and starfish.

Poison can be interpret as toxin presented inside organism. In which if the organism be ingested, it can cause devastating effect. Example would be pufferfish, shellfish and dinoflagellatus ostreopsis sp. (such as Gambierdius Toxicus).

The lectures, case presentation and practical session include proper first aid management on site of incident, recommendation of management in hospital setting, suggestion of post incident and dissemination of information for public awareness. There are also session discussing on the variation of animal venomous and poisonous animal, appropriate guidelines and references.
